FIRST VISIT

Please feel free to complete the following steps prior to your first appointment.  We can always help you once you’re in the office, so no pressure getting these done ahead of time!

Fill out a New Patient Form – No need to download, print, scan or e-mail.  Simply fill out the form online Before your first appointment, you’ll also receive an e-mail containing a link to the form.


 

Verify your Insurance Coverage – We’ll gladly verify your insurance ahead of time.  Please call us at (408) 357-0492 to provide your insurance information.

Greetings and Check-In – Enter our light-filled reception where you’ll find modern and stylish furnishings and greeted by our friendly team.  Sign in using our convenient iPad kiosk (or the traditional way if you prefer pen and pencil, of course).  Sorry – we don’t have a TV in our reception, because our patients shouldn’t have to wait long to be seen.  

Entertainment and Relaxation –  In the treatment room, put on a wireless headphone and catch up on your favorite Netflix show while we’re at work.  Heated massage chair and a warm blanket will help you forget that you’re at a dental office! When you’re done, freshen up with a scented hot towel. 

Treatment Plan and Education – We’re committed to empowering our patients with a clear understanding of their oral health conditions and treatment options.  We understand that most of our patients are not experts in dentistry, so we explain everything you need to know in detail  in plain English, so you can be in complete control of your choices.

We’re glad you enjoyed your first visit!  Just remember to ask your friends to mention your name when they come in, so you can be enrolled in our raffle for cool prizes.

INSURANCE & PAYMENTS

We work with ALL major PPO dental insurance plans.  We’re in-network with (Listed in alphabetical order) Aetna, Ameritas, Anthem – Blue Cross and Blue Shield, Careington, Cigna, DNoA – Dental Network of America, Delta Dental, GEHA – Connection Dental, Guardian, Humana, MetLife, Principal, Sun Life, UHC – United Health Care and more.  Don’t see the name of your insurance?  Please call or e-mail us, and we can help estimate the coverage through your insurance provider.

Depending on the insurance plan and the type of procedure, insurance companies reimburse anywhere from 50 to 100% of the cost.   We strongly encourage our patients to familiarize themselves with their insurance benefits, but we’re always happy to contact the insurance to help estimate the out-of-pocket expenses!

We understand that cost of treatment can be a barrier to dental care.  We will always be transparent and share detailed fee breakdowns prior to treatment whenever possible.

We accept credit cards (Visa, MasterCard, American Express and Discover) as a form of payment. 

We also offer various financing options (both third-party provided and in-house).

We understand dental emergencies don’t run on a schedule.  We’ll do our best to accommodate you by shifting existing appointments or staying late to help you get out of pain as quickly as possible.
